Different types of EHR have been developed by academic medical centers, the government, and industry.

Modern-day EHR first began to appear in 1960s. In 1967, Latter Day Saints Hospitals in Utah started using Health Evaluation through Logical programming (HELP) software. This software is notable for pioneering decision support features.

In 1969, Harvard Medical School developed its own software Computer Stored Ambulatory Record (COASTER) and Duke University began to developed The Medical Record (TMR).

In 1970, Lockheed developed Technicon Medical Information Management System / Technicon Data System (TDS). It was implemented at El Camion Hospital in California together with the groundbreaking Computer Provided Order Entry (CPOE) system. In 1979, Judith Faulkner developed the Chronicles data repository which later became Epic Systems.

Since the 1980s, more concerted efforts have been made to increase use of EHR. Developments include Veterans Health Information System and Technology Architecture (VistA) (originally known as Decentralized Hospital Computer Program (DHCP)) and Epic-Care (first Windows-based EHR software introduced in 1992).
